 Enrollment Process for CareLink Members

This page has been designed to assist you as you transition from CareLink into your new health plan, University Community Care Plan (UCCP) by Community First. Please use the information and resources below to learn more about UCCP.

What is University Community Care Plan?

University Community Care Plan (UCCP) is a health insurance plan managed by Community First Insurance Plans. UCCP offers expanded health benefits and coverage in addition to all other services you currently have through CareLink, including:

  • Expanded prescription drug coverage 
  • In-home urgent care
  • Emergency ambulance services
  • Hearing aids
  • Pediatric vision care for dependents
  • Durable Medical Equipment 
  • Chiropractic care
  • Hospice care

What is the difference between my current Carelink Plan and UCCP?

How do I complete my enrollment in UCCP?

Simply contact CareLink to verify your information, like your family’s gross income and household size. We can also help you add dependents (children) to your plan and find out if you qualify for a “premium tax credit” that could help lower your premium. You can contact CareLink at 210-358-3350, Monday through Friday, from 8:00 a.m. to 4:30 p.m.

Will I have to find new doctors as a UCCP Member?

No. UCCP and CareLink share the same network of doctors, specialists, and hospitals. This means you can continue to see all the same providers you see now as a CareLink member.

Does UCCP offer translation services?

Yes. UCCP Member Services Representatives speak both Spanish and English and there are free translation services available to you for your health care visits.

What will I have to pay to get health care services?

As a UCCP Member, you will experience cost savings when using your health care coverage. Some health care services may require a copay. A copay is a flat fee you pay each time you receive a certain service. However, after your copay is paid, you owe nothing.
