How To Get Your At-Home Over-The-Counter COVID-19 Test For Free
Beginning January 15, 2022, University Community Care Plan (UCCP) Members can be reimbursed for their purchase of at-home COVID-19 tests at local pharmacies, stores, or online. Tests must be authorized by the U.S Food and Drug Administration (FDA).
Submit a claim to be reimbursed by Navitus, your pharmacy benefits provider. Keep your receipt and complete the Navitus OTC-COVID 19 At Home Test Claim Form.
Please Note: reimbursement is for at-home COVID-19 tests only.
COVID-19 test kits will be covered at a reimbursement rate of up to $12 per test, with a limit of eight tests per covered Member every 30 days.
