Community First Care Plans help keep monthly payments lower, but members share more of the cost when they receive care. All Care Plans include a deductible and usually use a mix of copays and coinsurance.

Why Choose a Care Plan?

A Care Plan may be a good fit if you want a lower monthly premium and don’t mind paying more when you need care. Care plans are available in Bronze, Silver, and Gold levels.

How A Deductible Works

With a Care Plan, you may need to pay some health care costs before the plan starts helping pay for certain services.

After you meet your deductible, you may pay:

  • A copay for some services
  • Coinsurance for other services

Understanding Plan Levels

Bronze

Lower monthly payment, but higher costs when you get care.
On average, Community First pays 60%. You pay 40%.

Silver

A balance between monthly payments and health care costs.
On average, Community First pays 70%. You pay 30%.

Gold

Higher monthly payment, but lower costs when you need care.
On average, Community First pays 80%. You pay 20%.
