As our partner in care, we would like to inform our Provider network that University Community Care Plan by Community First has changed its name to Community First Mercado Plan.
This name change reflects the evolution of our Marketplace plan while maintaining our long-standing commitment to our Members, Providers, and the community we serve.
What This Means for Providers
While our name has changed, there are no changes to Members’ benefits, coverage, or provider network participation as a result of this transition.
Members will continue to:
- Use the same Member ID card
- Have access to the same covered benefits and services
- Visit their current in-network providers
- Receive care under their existing coverage
There are no changes to provider contracts, reimbursement arrangements, or claims submission processes associated with this name change.
New Member ID Cards for Plan Year 2027
For Plan Year 2027, Members will receive new Member ID cards reflecting the Community First Mercado Plan name.
These cards will be mailed to Members in December 2026. Providers may begin seeing the updated plan name during visits. We encourage front office and administrative staff to familiarize themselves with the new branding to help ensure a smooth experience for Members.
Members may continue to present their current ID cards until they receive their updated card. Eligibility and benefits should continue to be verified through your normal processes.
