Dear University Community Care Plan Member,

You’re receiving this letter because the Health Insurance Marketplace® informed us that in 2023 and 2024, you (or others in your household) used the premium tax credit, also called “financial help,” to help lower the cost of your University Community Care Plan premiums.

Every year that your household has Marketplace coverage and uses the premium tax credit, you must file federal income taxes and reconcile the premium tax credit you qualified for with the amount you used. You must complete IRS Form 8962 “Premium Tax Credit (PTC)” and include it with your return. IRS calls the amount you used the “Advance Payment of Premium Tax Credit” on this form.

Marketplace informed us that IRS records show you haven’t filed a federal income tax return or didn’t include Form 8962 to reconcile the premium tax credit for 2023 and 2024.

  • You must file these tax returns and reconcile the premium tax credit to continue getting this financial help to lower your Marketplace plan costs.
  • Act now or you’ll lose any financial help (premium tax credit or extra savings) you may otherwise qualify for in 2026. This means you’ll have to pay the full cost for your Marketplace plan premium and covered services in 2026.

What should I do next?
If you already filed 2023 and 2024 federal income tax returns with IRS Form 8962:

  1. Log into your Marketplace account on Healthcare.gov and update your 2026 Marketplace application by December 15, 2025 for coverage starting January 1. Make sure all your information (including household income) is correct.
  2. Check the box at the end of your application telling Marketplace you reconciled your premium tax
    credit for past years.

If you haven’t filed your 2023 and 2024 federal income tax returns or didn’t include IRS Form 8962:

  1. Log into your Marketplace account on Healthcare.gov to get a copy of your IRS tax Form 1095-A for both 2023 and 2024.
    • You can also call the Marketplace Call Center at 1-800-318-2596 to get a copy of the form. TTY users can call 1-855-889-4325. If you applied for coverage through a Marketplace enrollment partner, contact them for a copy of your IRS Form 1095-A.
  2. Use your Form 1095-A to fill out IRS Form 8962 to reconcile your 2023 and 2024 premium tax credit. You’ll fill out a separate form for each year and include it with that year’s tax filing.
  3. File your 2023 and 2024 federal income tax returns as soon as possible, even if you have an extension.
    • You must include Form 8962 when you file.
    • If you filed a federal income tax return for 2023 and 2024, but didn’t include Form 8962, you might need to file an amended federal income tax return (IRS Form 1040X).
    • Visit the IRS’ Interactive Tax Assistant at IRS.gov/help/ita or call the IRS at 1-866-682-7451, ext. 568 if you have questions about your household’s tax filing status for past years.
  4. After you file your 2023 and 2024 federal income tax returns with Form 8962, log into your Marketplace
    account and update your 2026 Marketplace application. Check the box at the end of your application telling Marketplace you reconciled your premium tax credit for past years.
